Patients are seen by appointment only and can be made during our regular office hours.
When you call, please leave your name and phone number, and an office staff member will contact you.
New Patients
If this is your first time to THIS practice, please bring the following with you:
- Printed and complete patient forms (found below)
- All current insurance cards including Prescription ID cards
- List of medications (Including over the counter, supplements, NSAIDS (Advil, Ibuprofen, Aleve, etc) with dosages and directions
- Photo ID
- A referral from your primary care physician if needed with your plan

Billing
Banerjee Kidney Center is currently in the process of being credentialed with multiple insurance companies. Please call your insurance carrier or office to see if we participate with your plan. If we do not participate with your insurance plan, your visit will be billed out of network. Please check with your insurance carrier to see if you have out of network benefits.
If your insurance requires a referral, please contact your primary care doctor 48-72 hours before your appointment to obtain the referral.
